Jayne Daylor, MSN, RN
Job title or function
Jayne Daylor MSN, RN is a Registered Nurse working as a Quality Improvement Consultant for the Rhode Island Department of Health providing clinical expertise related to heart disease, pre-diabetes, diabetes, hypertension; as well as related risk factors such as obesity physical inactivity, nutrition, and tobacco use. Jayne has over thirty years of nursing experience in Research and Practice Management and has spent the last ten years dedicated to practice transformation and quality improvement initiatives. As faculty for GLEARN, she provides coaching and training to new and experienced Nurse Care Mangers and assist them in developing skills to care for patients with complex care needs. Jayne holds a master’s degree from Boston College Connell School of Nursing.
Suzanne Herzberg
Job title or function
Suzanne Herzberg is the Director of Transformation for the Brown Primary Care Transformation Initiative at Kent Hospital. She facilitates quality improvement programs in federally qualified health centers, free clinics, and private practices. Current programs focus on increasing pediatric immunization rates, improving the care of people with diabetes and cardiovascular disease, screening pregnant women for anxiety and depression, and establishing relationships between pediatric practices and family visiting agencies. She has a doctorate in gerontology and a master’s degree in occupational therapy.
Casey Sardo, RD, LDN, CDOE
Job title or function
Casey joined Tri-County Community Action Agency as the WIC Director in the spring of 2018 and began offering Medical Nutrition Therapy and nutrition counseling at the Tri-County Health Centers in the fall of 2018. Casey attended Johnson & Whales University where she earned her Bachelor of Science Degree in clinical dietetics and will be finishing a Bachelor of Science Degree in nursing through Rhode Island College this upcoming December. She took the role of C+C+E Team lead in 2020 and has coordinated the project ever since.
Adrian Bishop
Adrian Bishop is Director of e-Health and Organizational Development at Advocates for Human Potential. Adrian has worked with Federally Qualified Health Centers for over 20 years. His specialties include Health IT, Payment Reform, Change Management, Workflow design, and Strategy Alignment. Adrian is a UDS reviewer and member of the national UDS leadership team. For the last 9 years Adrian has worked as a consultant with the Rhode Island DOH to utilize clinical data to improve population health outcomes and with the Rhode Island EOHSS on data aggregation and reporting projects.
Corey Goffinet
Job title or function
Corey Goffinet, Director of Quality & Health Information Technology (HIT) for Tri-County Community Action Agency and is a member of the Quality & Data Analytics Committee for Integrated Healthcare Partners (IHP) accountable care organization. He joined the Tri-County practice in October 2020 as the EHR Systems Developer and Data Analyst and was promoted to Director in the summer of 2022. As the Director of Quality and HIT for Tri-County Health Centers, Corey has an extensive knowledge of healthcare data systems, and utilizing those systems to produce meaningful reports that help drive quality improvement programs, policy, and workflows. He is an essential piece of the healthcare team that prioritizes patient needs and best practices at the core of everything he does.
